This is a NIGHT shiftrole, offering an additional $5.00 per hour float pool differentialPLUS our generous NIGHT Differentials on top of that! Managesindividualized, outcome oriented nursing care through the use ofthe nursing process. Provides care, treatment and services throughthe successful coordination and completion of a series of processesthat include assessment, planning care, providing care, inaccordance with departmental and hospital policies andprocedures

1 -ASSESSMENT/REASSESSMENT
Assesses and diagnoses patientand family needs in order to provide quality care to assignedpatients by:

a -Initiating admission assessment within 8 hours of admission,identifies and documents patient/family/significant other teachingneeds upon admission anf throughouthospitalization.

2- PLAN OF CARE
Develops, discusses,and communicates a prioritized problem list/plan of care for eachpatient, in collaboration with designated support persons in orderto address all identifiedneeds.

a - Develop,evaluate and update individualized plan of care for patients anddocumentsd outcomes.

b - Activelycommunicates plan of care with the primarynure assigned to care forthe patient

c - Facilitatesdischasrge/transfers by goal of 12:00 pm, communicating barriersutilizing the Chain of Command.

d -Documents all patient care follwoing the department of nursingpolicies, unit based standards and nurse sensitive indicators(falls, skin pain).

e -Assessess/re/assesses and documentsd patient's level of comfortutilizong the appropriate pan scale and the patient's response toeach intervention both pharmacoligical and non-pharmacological perSouth Shore Hospital Policy.

f -Works on behalf of patient/family. Seeks help to repersentpatient/family when they are unable to representthemselves.

g - Raises ethicaquestions and concerns with clinica team. Seeks available resourcesto help formulate and understand ethicaldecisions.

3- MEDICATIONADMINISTRATION

a -*Demonstrates knowledge of and follows SSH policies and proceduresfor administering, transcribing, and recordingmedications.

b - Demonstrates properporcedure for the documentation of narcotic withdrawal,administration, verifies count, wastes per policy and resolvesnarcotic discrepency.

c - Initiatesand maintains IV access according to SSH policy andprocedure.

6- SAFETY
Fosters a "Culture of Safety" throughpersonal ownership and commitment to a safeenvironment.

a - Verifiespatient identification with two idenitifiers prior to the start ogany invasive procedure, including "time out", admisinstration ogcare, medications, labeleld specimens and documents confirming thecorrect patient, procedure, site, equipment andconsent.

b - Complies with thecurrent CDC hand hygiene guidelines through proper handwashing.Adheres t universal precautions, makes approriate use of personalprotective equipment at all times and appropriately disposes ofhazardous materials. Maintains awareness of MSDS sheets and how toaccess them.

c - Assesses eachpatient's risk for falling throughout hospitalization usingappropriate interventions to preventfalls.

d - Ensures environment meetsregulatory requirements at all times (ie: no cardboard boxes onfloor, equipment cords are secured and plugged into foot of bed,etc .).

e - Understands and is ableto demonstrate individual roles and responsibilities in teh eventof hospital codes/emergencypreparedness.

f - Practices patienthand off utilizing the principles of SBAR - including walkingrounds.

7 -PROFESSIONAL DEVELOPMENT
Assumes overallresponsibility for own professional development by incorporatingevidenced-based practice, research, and performance improvementinitiatives as a part of ongoing nursingpractice.

a - Obtains atleast 5 contact hours per year in area ofpractice.

b - Practices within thelegal boundaries of MA Nurse Practice Act. Directs other licensedand non-licensed personnel asssigned.

c - Active participant inunit based initiatives and/or hospitalcommittees.

d - Understands anddemonstrates job responsibilities for charge nurse, preceptor andmentor role.

e - Analyzes cause ofproblems, identifies and evaluates alternative solutions, andselects appropriate solutions and communicates appropriately andeffectively utilizing the proper Chain ofCommand.

f - Assumes and active rolein keeping informed about changes in hospital policy, procedure andequipment.

g - Interacts withpatient and families on a compassionate and humanistic manner -demonstrating empathy, sensitivity andpatience.

8- TECHNOLOGY
Embraces technological solutionsto work processes andpractices.

a - AccessLotus Notes to review e-mail, learning management and otherresources as applicable to RN role.

b - Utilizes all Meditech applications required by department andunit standards (PCS,EMR, Order Entry,etc .)
