Colorado Permanente Medical Group is searching for an Internal Medicine or Family Medicine physician preferably with a geriatrics fellowship to be a leader in our new Complete Care Physician role. If not board certified in Geriatric Medicine, completion of the UCLA Intensive Course in Geriatric Medicine & Board Review, or similar training, should be completed within the first year of hire.
Complete Care Physicians specialize in the care of adult members with high needs due to a complex array of medical, social, psychological, and behavioral factors. These Primary Care doctors focus on matching care to the needs of the individual patient using an interdisciplinary approach focused on increased continuity access, panel management, and care coordination. A comprehensive team of healthcare professionals will ensure the well-being of members by handling a range of services including on-site and virtual primary care, chronic and acute care management, coordination with relevant specialists, and after-hours call coverage. Experience and/or formal training in complex care, geriatrics, and team-based approaches is preferred. Due to the high patient complexity, target panel size for Complete Care physicians will be 600 members per 1.0 FTE.
This is a 5 day a week job, including Saturdays, based on 1.0 FTE with some of that work encompassing virtual call coverage from the hours of 8AM to 8PM.
